Something interesting about yourself that you would like to share: I enjoy baseball, running, biking and crossfit. I am a husband and dad to three amazing little people.
Describe Your Role/Day In the Life: As one of the Transition of Care Team leads, I start my day early to get a head start on the day. Gathering patient data, labs, diagnosis and any new info involving the patients on a busy 36 bed unit. The primary diagnosis of most of these patients is Congestive Heart Failure. I work/precept one to three student pharmacists, interns and/or residents. I make it a priority to ensure we work as a team, communicating with the staff RNs on the floor, advanced practice providers, physicians, case management and social work to care for these patients. Responsibilities include reviewing medication reconciliation on admission and prior to discharge to ensure the most accurate information is provided, as well as routine orders, pharmacokinetics and anticoagulation dosing.
At Ascension St. John we have the opportunity to be on the units, as we have a decentralized pharmacy. We are accessible to the nursing staff and providers for any questions and clarifications, as well as have several opportunities to provide patient education in person.
Why are you an MSHP/MPA Member: I originally joined MPA to keep up-to-date on pharmacy policy going on in the state. Once I became a member, I began to see how important it is to be involved. I feel to effectively make change, one has to be a part of that change. I have watched many of my MPA colleagues throughout the years and admire the actions we have taken to improve pharmacy policy and practice in the state of Michigan.
